This is the "BoardPolicy" template. You can use this template on pages that represent "official" board policy. This means the policy was voted on by the board at the time, and that it was communicated to all members, probably on the members@ mailing list. Pages with this template are automatically added to Category:BoardPolicy and should be protected after tagging.
It should be called in the following format:
